Entries give name, sex, and date of death of child; name and location of hospital; names, ages, and addresses of parents; number of children previously born to mother; method of delivery; cause of death; and any arrangements. Arranged chronologically by year and month, then alphabetically by county, and then alphabetically by name of child. Access is restricted by law for 20 years after date of death.
